Welcome to the FAQ page. Hardee DeSoto Community Health Workers Program FAQs cover everything from application questions to requirements.   • Do I need to be sick to be able to join the HD CHW program?
    No, you do not need to be sick or have an illness to be able to join the HD CHW program.
  • What are the requirements to join the HD CHW program?
    Currently, the only requirement to be able to join the program is that the client lives in Hardee or DeSoto County.
  • What cities fall within Hardee and DeSoto County?
    Hardee County Cities are Wauchula, Zolfo Springs, Bowling Green, Ona, Limestone, and Gardener. DeSoto County Cities are Arcadia, Fort Ogden, and Nocatee.
  • How can I apply to the program?
    To apply for the HD CHW Program, give us a call or reach out via email. We will have one of our CHWs reach out to set up an intake appointment.
